Financial Policy
Thank you for choosing Five Forks Dental Care, Inc.! Our primary mission is to deliver the best and most comprehensive care available. It is important to us that the quality of our business services matches the quality of our dental care. We want the handling of your account, from the start, to be perceived as an extension of the dental care we provide you and your family. We believe the best way to save our patients money is to do our best job the first time.
All appointments are exclusively reserved:
Unlike most offices where a patient is scheduled for one filling at a time, we recognize that your time is valuable. The more that we can get done in one visit the less time you will miss from work and family. We are happy to offer long appointments and dedicate our time exclusively for you. Subsequently, we require a two day notice for any change in scheduling for short visits and a one-week notice for larger, more comprehensive treatment plans. We reserve the right to charge a fee of $75 per hour of reserved treatment time with our doctors for any changes in scheduling without a sufficient notice. All appointments are to be handled by our front office team during our regular business hours.
Patient's role:
As with any partnership, both parties have a role to play. Our role is to provide you with the best quality service and most comprehensive care available. In turn, your role is making sure you show up for the appointments that you schedule and pay for your treatment at the time services are rendered. Our team will work with you to determine financial arrangements that make sense for both of us. With an agreement made, our joint follow-through will result in a win for everyone.
Regarding Insurance:
Our insurance experts work with you to maximize your insurance benefits. We file insurance claims and accept assignment of insurance benefits. So that we may file your insurance claims correctly, we ask all patients to complete our Information and Insurance Form before seeing the doctor as that ensures our office of obtaining the correct information to better serve you in regards to your benefits. Patients are ultimately responsible for the agreed upon fee regardless of insurance benefits. No insurance? No problem! Check out our In-Office Savings Plan
Payment Options:
An important part of our mission is making the cost of optimal care as easy and manageable as possible for our patients by offering several payment options:
Cash, Check, Visa, MasterCard, Discover Card or American Express, or a selected payment plan.
Plans from Care Credit, Lending Club, Citi Financial, GE Financial or one of our other major financing companies will allow you to pay over time with little or no interest. Convenient, low monthly payment plans are also available with no annual fees or pre-payment penalties on most plans. Excellent credit is not necessary for approval.

Dr. Hapney and her team go above and beyond to make you feel like a family member and not just another person coming into their office. They really take the time to explain in detail any treatment that is needed. The first visit was a little longer because they took the time to do a through exam of the health of my gums and teeth but I felt like I had been educated on what was going on and needed to be done.
The Spa offers the best facials and chemical peels. Shawna is excellent at helping you with skin care as well.
